Fill out both the Player/Cheerleader Contract and the Parent Code of Conduct and mail to the address below along with following items:
- Copy of birth certificate
- Copy of utility bill
- Check(s)
You will be required to get a physical for your child.
If your would like to volunteer to Coach or become a General Manger for a team

$300 fee for all players
$100 refundable deposit for equipment and uniform
